CBD Application Slovakia: Submitting Your Application

   Please read all of the following information carefully.

1. You will need TWO official apostilled background checks when applying in Slovakia. 
2. You will need TWO unofficial copies of ALL documents - for EVERY applicant.
3. Appointment Requirements: Do not make travel arrangements to Slovakia solely for the purpose of attending an appointment until you have a confirmed booking. The Slovak appointment scheduling system does not allow us to select from a calendar of dates and times; instead, it automatically offers the earliest available appointment. This is frequently within seven (7) days. Accordingly, applicants must be prepared to travel on short notice, with as little as one week’s advance notice. 
4. Appointment Duration and Locations: Appointments in Slovakia typically require attendance over multiple days—generally one appointment on the first day and a second appointment on a different day. You should not expect to complete all submissions in a single day. Travel is required to our current partner offices in Zilina, Banská Bystrica, and Košice. We do not service Bratislava or Trnava.
5. Expected time in Slovakia: As little as 4 days, but as many as 10, depending on appointment location and availability.
6. How many people are applying?
   1. If more than just yourself, you must include their name(s), date(s) of birth, and passport number(s) at the bottom of this form where it asks "Do you have any questions at this time?"
7. Availability Information: When completing this form, you must provide specific date ranges for your availability. This way, we can keep an eye on your upcoming availability. Examples include:
   1. “The week of August 25th”
   2. “The entire month of October”
   3. “Anytime in March or April”
8. Fees and Payment Terms:
   The service fees are as follows:
   1. Scheduling and coordination of appointments & document review: 400€, which includes the scheduling and coordination of your appointments, administrative tasks associated with the submission process in Slovakia,  application/forms completion assistance, and final document review. If you are NOT a full-service consulting client of Jamieson Consulting Firm s.r.o., a non-refundable 50% (200€) deposit is due at the time you submit your request for an appointment. The remaining balance will be due once your appointments have been scheduled. A 50€ discount is applied for full-service clients of Jamieson Consulting Firm s.r.o. 
   2. In-person interpretation services are charged per appointment: 300€ per appointment in Košice and 400€ per appointment in Banská Bystrica or Žilina, reflecting the additional travel distance for the interpreter, who is based in Stará Ľubovňa. Each appointment fee covers travel time and up to 1.5 hours of total time, whether spent waiting or actively interpreting. Any additional time is billed at 30€ per additional 30 minutes.
   3. Please note that there are two separate appointments when submitting in Slovakia. These appointments are typically scheduled on different days, usually within one or two days of each other. Because the length and nature of each appointment can vary, we are unable to provide an estimate of the total final cost in advance. The initial interpretation fee (600€ for Košice or 800€ for Banská Bystrica/Žilina) is due once your appointments are confirmed. Any additional time fees are payable at the time the services are rendered.
   4. There is an additional service fee to compile your document packets and/or make copies of your documents for you, based on time and materials used. This will be included in the invoice you receive from the interpreter.
9. All fees are non-refundable. The exact interpretation fee is determined by the number of days and hours the interpreter will be needed. 
10. Travel Insurance and Liability:
    It is strongly recommended that you obtain travel insurance with “cancel for any reason” coverage. We are not responsible for:
    1. Canceled or rescheduled appointments
    2. Interrupted or delayed travel
    3. Extended stays or related expenses
11. We are not government officials, lawyers, or attorneys, and we do not provide legal representation.
12. Document Review & Appointment Notification:
    1. Once your deposit has been received, you will receive a link to the client portal. Here, you will upload your documents for review and find the necessary applications and documents for completion. 
    2. Once an appointment has been made within the timeframe you've provided, you will be contacted.
